ELIGIBILITY CRITERIA FOR ADMISSION
A student has to appear in state AYUSH counselling after qualifying the NEET exam. After allotment he/she has to deposit his/her original and one set self-attested photocopy of documents given below –

| 3 | – | Print Out of Registration for UP AYUSH UG counselling |
| 4 | – | Allotment Letter |
| 5 | – | Mark sheet of High School or Equivalent Examination |
| 6 | – | Certificate of High School or Equivalent Examination |
| 7 | – | Mark sheet of Intermediate or Equivalent Examination |
| 8 | – | Certificate of Intermediate or Equivalent Examination |
| 9 | – | Character Certificate of last institute |
| 10 | – | Transfer Certificate |
| 11 | – | Migration Certificate |
| 12 | – | Domicile Certificate (applicable for those students, who have passed High School or Intermediate or both from out of UP or AMU Aligarh) |
| 13 | – | Certificate for Reservation in case of SC, ST, OBC, EWS or any other reservation |
| 14 | – | Gap Certificate (Notarized Affidavit on RS 10 Non-Judicial Stamp) |
| 15 | – | Undertaking by Student for Anti-ragging on www.antiragging.in |
| 16 | – | Undertaking by Parents/Guardian for Anti-ragging on www.antiragging.in |
| 17 | – | NSC of RS 4000/- as security money in the name of Student. (Refundable) |
| 18 | – | Photo ID Proof (Driving License/Bank Pass Book with Photo/PAN/Passport/Photo ID issued by last Institution) |
| 19 | – | 04 Latest Passport sized colored Photographs |
